Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Ok, glad you figured out that one but sorry you ran into another. But logging into your account just now, without changing any settings on the Web-to-Lead Setup page, I'm able to uncheck the box just fine. See the screencap below:
So not sure what's happening with yours. Maybe post a screencap of what you are seeing?
Thank you! I had been typing my email into the email template as well so that's why the template wasn't showing up. I did run into another problem though on step 101- I don't have the option to uncheck "enable spam filtering." Could you help me on this as well?
Emily I just tried it in your account and found the template, no problem. Here's a screencap from your account:

Well, you would have created that in Steps 58-73. And you would have given it that name in Step 65. Are you sure you did those steps?
If so then, I'll look into your account and see what I can find but you'll need to do these 3 steps first so I can access your account:
1) Log in and click on you name (upper right) and click My Settings
2) Click Personal (upper left) and then Grant Account Login Access below that
3) On the row for The Max Labs Project Support, use the --No Access-- pull-down menu to select 1 Month and click Save
