Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Ok well then the URL is correct now but there’s some problem with your Flow. Adding a lead is triggering it which is correct but the flow is failing so the lead add process is failing.
