Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.
Before you post make sure you check the Help FAQ for an instant solution first. Then, if you can't find it there, you can post your problem here and we'll get to it ASAP (within 24 hours), but be sure you:

Acutally it did work. I just logged into your account and saw the leads you added - one with "Syod" and one with "Syoyd". I guess you tried twice.
The screen in your browser that you're screencapping is supposed to just be blank after you enter the command. Note this in the instructions say:
"The page will just go blank but you just added a Lead record to your SF org by issuing your own API request directly to SF’s little “webtolead” servlet, somewhere in their massive Web cloud."
So to see that it worked, you got to your Leads tab and pull down the listview menu and choose Today's Leads and you'll see them.
it did not work
Ok, well, look at what it says after 102 for Max Flanagan's account. It ends with:
oid=00D4P000000yb1T&first_name=Magan
She's telling it her Org ID number (account number) and then telling it that the field "first_name" should be set to "Magan" which is the first two letters of her first name and the last three letters of her last name.
So you, you need to follow your Org ID number with "&first_name=Syoyd", right?
