Well, looking at your account won't help. The problem is with the HTML you're generating in that box that doesn't stay after you copy/paste it. So I wouldn't see anything that would help.

It looks like you're either skipping a bunch of those steps, like 91-96, where you take out City & State and put in Mobile, Bay Area Location and Investment Potential. What you're showing is just the default fields with none of those changes made.

So try again and this time make sure, at the end of Step 98, that your screen looks exactly like the screencap shown after Step 98, showing First Name, Last Name, Company, Email, Mobile, Bay Area Location and Investment Potential in that list of Selected Fields (with the red arrow pointing to it.)

If yours looks the same, then the HTML generated when you click Generate will be correct. If you say you tried this 3 times and got the same thing everytime, and you're sure you did every step, then I suspect you're getting confused between the 3 HTML files you produced and you're still using the first one where you got things messed up somehow.

This time, instead of naming it "myweb2lead.html" use a different name, like mynewweb3lead.html or something and then make sure that is the file you open in your browser to see the form, ok?

I am so confused. I redid the TextEdit steps like three times and all the categories are still not there like in the picture under step 137. See attachment.

When you log back in, just go to Setup if you're not there already and then do Steps 89-98 and that will generate that code again for you.
