Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Seems like a browser security setting. Try downloading Firefox or some other browser you don’t already have and see if that helps. Could be, when you copy/pasted the HTML, you cut off the bottom where the Submit button code is located. Let me know, ok?
