Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Yeah, it looks like you missed Steps 89-97 somehow. Those are where you remove some of those fields from the form and add the others like Bay Area Location and Investment Potential. Anyway it's easy to fix. Just do Steps 89-98 now and then copy all the HTML it generates and put that into a new file, maybe call it "myWeb2LeadNew.html" or something and then open that file in your browser and the correct fields should be there then.
