Is there a way to get this screenshot with our profile information ? Doesn't allow for the window in the top right to come up with my name.

Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

You don't need to. The screen you are showing is the form you open to create a new Pitch record. You fill it out and click Save. And then, right after you save it (or later on, if you open up the record to view it), it will display as a regular screen and you can then open your identity drop-down window at the same time.
