Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

It looks like those 87 through 90 (at least) have already been done. If those fields have been taken out of the layout already you can't take them out again.
Are you trying to redo the steps after doing them already? That's never necessary and rarely works. The cover sheet warns against that. If you have a problem somewhere, we need to fix that right there and go on. Repeating everything's not an option.
What did you run into that made you try to repeat everything?
