Steps 56-67; The screenshot example shows 5 fields after clicking save. I still have 10 fields? How do I fix?

Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Well if you look at Step 66, it just lists 5 of the fields to move over. To fix this, do Step 65 so you are seeing the popup with the Available Fields on the left and the Visible fields on the right. Then for each field that doesn't belong in Visible fields, click to highlight it and then use the left-arrow to move it back over to Available Fields. Then Save.
