Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Ok, well it looks like you repeated the import of the data a couple of times and wound up with lots of extra Pitch records.
You can delete all the ones above P-009 if you want to and then continue on.
