On Step 81 it says we hit the "New" button and a form is supposed to appear but nothing showed up when I tried it. I even created a new account and went through all the steps more carefully and slowly but I still had the same result. My lab is due tomorrow, please help!

Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Sorry you're having trouble, Claire, but I'm sure we can fix it in time for your deadline. My guess is that you have some tight security settings that is blocking the popup window with the form but first let's make sure you're clicking the right "New" button. Yours should look like this one below but without the Pitch records, of course:
Is that the New button you're clicking? If so, then it should show the popup directly in front after you click. So then it's probably your popup window settings. Those can be found (& changed) in different places, depending on your browser and which version you're using. For the latest version of Safari, it would look like this:
Notice there is a place to set the action for specific sites that are open and another place below, for all other websites. Try setting those to "allow" and see if that makes a difference.
If you're still having trouble, try downloading/installing another browser, like Chrome or Firefox (both free). Since you would be installing them freshly, the settings likely wouldn't be blocking the popups yet but you might need to play with their settings too. If you have some special security software on your Mac, that also might be interfering with popups.
As a last resort, is there a computer lab or library where you could go log into your account and just try that step on another computer? At least that would tell us that it is some setting on your Mac that is blocking things.
