Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Pitch ID is assigned automatically by Salesforce so they don't make it a slot in the form where you enter the data into the fields of a record. Just go ahead and fill them in and click Save and then you should see one appear in the All list view.
